Documents required for the import of medical devices
- Application letter
- Sales contract – copy
- Invoice – copy
- Certificate of Origin – copy
- Quality Management System Certificates – copies (ISO 13485 or CE 93/42).
- Electronic version of invoice (in MS Excel format) should be sent to [email protected] email address (Name of the both parties as well as invoice numbers should be mentioned in the topic)
- ID – copy (for individuals)
- VÖEN – copy (for individuals)
All copies presented to our center should be certified with stamp and signature of the applicant.

- Power of attorney (for representatives and individuals)
- Invoices – 2 copies
